Banner Network Colorado Enters Into Value-Based Agreement with Humana

Banner Network Colorado (BNC), an integrated provider delivery network operating in Northern Colorado, and Humana Inc. (NYSE: HUM), a leading health and well-being company, announce a new Accountable Care agreement covering individuals and families purchasing insurance in Weld and Larimer counties through Connect for Health Colorado and through Humana commercial health maintenance organization (HMO) health plans for small employers.

The new Humana-Banner network Accountable Care agreement, effective January 1, 2015, will provide highly coordinated care and a world-class patient experience to those choosing Humana’s (HMO) plans through, but not limited to, Colorado’s insurance marketplace, which continues enrollment through February 15, 2015.

The arrangement will offer more personalized care and wellness programs focusing on individual preventive care, chronic conditions and disease management.

Under terms of the Accountable Care agreement, Humana members will have in-network access to Banner Network Colorado’s more than 300 primary care physicians and specialists, as well as to Banner Health’s local hospitals - North Colorado Medical Center in Greeley, McKee Medical Center in Loveland and Banner Fort Collins Medical Center (opening in April 2015) - its health centers and clinics, and other Banner ancillary services.

The value-based arrangement emphasizes quality outcomes with a focus on care coordination and leverages measures as defined by the National Committee for Quality Assurance (NCQA) Healthcare Effectiveness Data and Information Set (HEDIS) like; diabetes care and treatment, breast cancer screenings, colorectal cancer screenings and high-risk medication.
